How to Create and Edit Evaluation Forms
Go to Settings > Forms
Select the form type you want to create. Evaluation forms fall either under the Activity Overall Evaluation or Session Evaluation .
- On RSS and Enduring Materials, session evaluations are attached to the Sessions. These two activity types do not use the Activity Overall Evaluation.
- All other activity type have both Overall Activity Evaluation and Session Evaluation, both of which are attached to the activity on the Activity information page.
Look for the Master Evaluation form that you desire to duplicate.
Click on Duplicate Form.
Note: When you duplicate a form make sure you match the title of the form to the title of the Activity for which you are creating the form. Try to remove the words ‘duplicate’ and ‘Master’ from the new form. This way you would not have several Master forms and you would not have any confusion in picking the right Master form when you have to duplicate for other activities.
Once a form is duplicated you can edit and add questions; edit and add answers as per the specific activity and session needs.
1. If the type of question you want to add already exists, you can duplicate the question, then edit the question and change the display order
a. After locating the question you want, choose DUPLICATE QUESTION
b. Then locate the duplicated question (at the bottom of the evaluation) and edit as needed
c. Paste question into NAME and QUESTION boxes and adjust Display Order – change to make it flow as much as possible (See answer order at end of each question and put in numerical order)
d. Click SAVE
e. As necessary, edit and/or add the answers
2. If this is a permanent change, the MASTER needs to be updated. Changing the master will only change evaluations going forward. If you have evaluations already created for upcoming lectures or activities that haven’t been used yet, you need to go into those evaluations and make the same change.
NOTE: If you make a mistake, you can’t delete the question. You need to click EDIT and make it INACTIVE
Note: Deactivate On date on the Edit form page should correspond with the respective activity/session credit claiming end date.
